How to access the service

There are several ways for you to access our service:

  • You can ask your GP to refer you, or they may suggest this themselves
  • If you are seeing a consultant or midwife they can refer you
  • You can complete a self referral form. The self referral form can be posted to the address included on the form or you can email it to nwangliaft.centralreferralteam@nhs.net

Self-referral

To complete a self referral to the Perinatal Pelvic Health Service (PPHS) please click here. This service is available to you if you are pregnant or had a baby in the last 12 months.


The PPHS accept referrals for the following pelvic health problems; leaking of urine (pee) and/or stool (poo), pelvic organ prolapse (bulging or heaviness in the vagina) or injuries in the area between the vagina and anus. 

Please only complete this form if your pregnancy was, or is under the care of North West Anglia NHS Foundation Trust (Peterborough City Hospital or Hinchingbrooke Hospital). 

Please complete all sections of this referral form to the best of your ability. This will ensure it is triaged correctly by a member of the team, and your care is arranged correctly for your pelvic health concern.  

Once you have completed the form please email the form to nwangliaft.centralreferralteam@nhs.net or alternatively post it to our address which is included on the form.
