Going Home from Hospital

When patients are well enough to leave hospital, and if needed, we will ensure they have the right support and care to continue their recovery at home. 

This will help them:

  • Get better faster
  • Stop muscles becoming weak from staying in bed too long
  • Avoid picking up infections from hospital
  • Be as independent as possible

We will arrange for a home visit to understand what help and support patients may need. Everyone's needs are different, and we'll make sure patients get the support that's right for them.

To do this we will need to share patient information with carefully selected partner organisations who will provide support at home. We will make sure patients are happy for us to do this and will not share information without permission. Patients can say no if they do not want information shared, and we will only share information that is needed and with the right people.
