When patients are well enough to leave hospital, and if needed, we will ensure they have the right support and care to continue their recovery at home.
This will help them:
We will arrange for a home visit to understand what help and support patients may need. Everyone's needs are different, and we'll make sure patients get the support that's right for them.
This may include:
|
Helping get home ready for their return |
|
| Helping with washing, dressing, and personal care | |
| Helping with medicines and reminders | |
| Helping to moving around safely | |
| Helping with essential household tasks, shopping and heating checks | |
| Preparing meals and snacks | |
| Providing exercises to help improve strength and movement | |
| Providing Special equipment to help at home (for example, grab rails or walking aids) | |
| Sharing information and advice about local community groups and voluntary organisations that can provide support |
To do this we will need to share patient information with carefully selected partner organisations who will provide support at home. We will make sure patients are happy for us to do this and will not share information without permission. Patients can say no if they do not want information shared, and we will only share information that is needed and with the right people.
